Permission Form Allowing PSA to use Images

INSTRUCTIONS:

NOTE: This form is valid for three (3) years from the signature date. It covers all works submitted to PSA by the maker. It is not necessary to complete a form for each image.

1. Open the form (.doc format) by clicking here.
2. Print out the form, but consider typing your name in the Name (print) field first.
3. Sign and date the form.
4. Take a photo of the completed form with your cellphone. Scanning it into a pdf or jpeg file is also fine.
5. Email the image (jpeg or pdf type files only please!) to psa@wasatchcameraclub.com
6. The Club PSA Representative will verify completion and upload the completed forms to PSA.
7. PSA may request model and/or property releases to publish some images.
